Home Health Care/Private Duty Care
HHAs, CNAs & Companions
Do you need short-term assistance in your home following a hospital visit? As a member of the Private Care Association, Aurora's private duty RNs and LPNs will provide care assessment and implement a healing protocol for the continuity of care. In addition, we provide CNAs & Home Health Care Aides (HHAs) to assist with routine home care including light housekeeping, meal preparations, bathing, dressing, grooming and provide companionship.
Health Care Concierge Programs
Do you have your own personal health care concierge working for you? At Aurora, we assist you in navigating through the ins-and-outs of the health care system - while building a strategy to achieve optimum health and well being.
We facilitate all care including: communication between multiple physicians, offer physician referrals, schedule doctor's appointments, research treatment options, provide medication management, compile medical records, oversee care staffing and available to plan and accompany the patient on social functions.
Elite Nursing Assistants Programs
For the most challenging of care situations, and for patients seeking superior care specialists, Aurora's Elite Nursing Assistants Team is available on a contract basis. We have been retained by corporate executives and business professionals of the like to provide in-home care services.
Pre/Post-Operative Care Programs
With elective and cosmetic surgery cases more prevalent today, Aurora's nurses aides are available pre/post-surgery to provide safe transportation to and from your home and the surgical facility, and to assist with daily home care and rehabilitation with descretion.
Case Management Programs
Aurora case managers will provide a turn-key solution to plan and manage the patient's physical and social environment when long-term care is required.
Case management involves patient assessment & care planning, overseeing caregivers, collaboration with physicians, consultation with families, provide crisis intervention, and address health insurance and medical-related financial issues.
Geriatric Care Management
Today, more and more families are turning to a Geriatric Care Manager who specializes in assisting the elderly with long-term care planning. We are a member of the National Association of Geriatric Care Managers and our GCMs are trained in gerontology, social work and nursing, and offer counseling and resource support. Aurora's GCMs will assist with identifying housing & transportation, home care aides, and senior care facility placement.
Speciality Services
In addition to our Elite Nurses Assistants Team, we provide nurses, aides and companions to escort our clients on private jet charters for emergency and non-emergency travel.
We also provide pediatric caregiver services to assist families with the care of infants and children following short and long-term hospital stays.
Aurora Nurses has aligned with several highly-trained rehabilitation professionals including Physical, Occupational and Speech Therapists, and Dieticians who assess and design personalized execise, safety, speech and nutrition programs for our clients. It is our objective to return each client to their highest level of function and performance.
Payment for Services
Fees for Private Duty/Private Pay Services are determined by the type of caregivers assigned and the term of service of each patient's case.
